Employee Submission
Your team members can submit a report by phone or online*. In both methods, they will leave a summary of the incident and contact information via voicemail or the online form.
*A company code will be required for online reporting.
Sun Up Review
The Sun Up team will review the submission to determine a proper course of action.
1. Contact team member for more information.
2. Submit redacted summary to the company contact.
3. Call the company contact for a debrief of the incident.
Company Report Sent
The designated company contact will recieve a written summary of the incident. The report will be redacted if there is no violation of federal or local laws.
Extended Support Engaged
Optional
If requested, the Sun Up team will support the investigation or navigation of sensitive situations in collaboration with the company contact or designee.
